All of the following items are needed for registration:
- Your child's Birth Certificate Official record of birth
- Your child's Social Security Card (optional
- Current Physical Examination (dated within 12 months prior to registration)
- Immunization records, which must be up to date (680 Form)
- Proof of Address (F.P.L. bill, lease, water bill)
